The program

At Hiya Health, we are aiming to help people with pre-diabetes to reduce their risk of developing Type 2 Diabetes and help those with Type 2 Diabetes Mellitus achieve remission. We will focus on changes in dietary, lifestyle and exercise routines to help improve your blood sugar levels. We understand there is a lot of conflicting information about how to best manage diabetes. That’s why, with expert guidance from our accredited dietitian and exercise physiologist, you can trust that you’ll receive the most up-to-date, evidence-based advice tailored to diabetes management.

This 4-month program includes fortnightly 1-hour group education sessions focused on diet and exercise to improve blood sugar management. Participants will also receive an initial 45-minute session with a dietitian. Medicare rebates are available for eligible individuals for both the group sessions and the initial dietetic consultation.

Who can participate in the program?

If you’ve been told you’re pre-diabetic or have been diagnosed with Type 2 Diabetes, whether recently or years ago, this program is for you. Our specialised Type 2 Diabetes program is designed to help you manage and improve your blood sugar levels. With the combined expertise of a dietitian and exercise physiologist, we will focus on dietary changes, including carbohydrate types and portions, along with exercise for better diabetes management. The group education sessions will cover general strategies for managing diabetes, while one-on-one consultations are available for a more personalised approach to diet and exercise.

You will receive evidence-based information on improving blood sugar management, along with personalised diet and exercise plans available through one-on-one consultations with both our dietitian and exercise physiologist. 

Assessment tools

 At the start of your program, we will ask for blood tests to establish a baseline for your HbA1c and fasting/random glucose levels. These tests will be repeated two weeks before completing the program to assess changes and measure the success of your blood sugar management.

Group Education Session: The main part of the program is attending the 60 minute group education sessions that are focused on evidence-based diabetes management. These sessions include specific topics covering both diet and exercise in the importance of managing blood sugar levels and reducing our risk of diabetes. 

Free Diabetes E-Book: Each client attending the diabetes program will be provided with an informative e-book that will cover important information on diabetes management that will also reflect the topics being covered in the group education session.
